Physicians and providers can fax referrals to 713-665-1938, email them to us at Office@primekidstherapy.com, or follow the link below to complete our online referral form. Once a referral is received, we will contact the patient to conduct the intake and set up the initial appointment. After completing the evaluation, our office will fax or email the entire evaluation back to the physician or referring medical provider for review and approval of the plan of care. We will start providing therapy services only after we receive the physician or referral medical provider’s approval.
