Refer A Child

Physicians and healthcare providers may submit referrals by fax to 7713-665-1938, by email to Referrals@primekidstherapy.com, or by completing our secure online referral form. Once the referral is received, our team will contact the family to complete the intake process and schedule the initial evaluation. After the evaluation is completed, a full report will be sent to the referring provider via fax or email for review and approval of the plan of care.
